This program includes four key services designed to support medication adjustments in collaboration with healthcare practitioners:
- Primary Assessment An initial evaluation to understand the client’s current medication regimen and recovery objectives.
- Treating Party Contact (2 sessions) Direct communication with the client’s healthcare providers to discuss and coordinate necessary medication changes.
- Progress Update Report A comprehensive report detailing the client’s progress, medication adjustments and recommendations for ongoing management.
All services are pre-approved with no upfront costs, and billing occurs upon completion of each service.
