Initial Pharmacy Review with Medication Management
At the outset, assess treatment appropriateness, current medications, interactions and risks. Establish baseline symptoms, function and treatment goals, and coordinate the medication management plan with the treating team.
Outcome: An initial assessment and clear plan for measuring clinical benefit during the trial.
Medication Monitoring to Determine Outcomes
At the midpoint, around week 6, assess progress against the agreed goals. Review clinical benefit, adverse effects, adherence, dosage and medication-related risks, with treating provider liaison where needed.
Outcome: A progress assessment to guide whether the trial should continue, be modified or be reconsidered.
Final Pharmacy Review to Determine Effectiveness
At week 12, compare outcomes with the initial assessment. Determine treatment effectiveness and assess whether ongoing treatment is clinically justified, reasonable and necessary.
Outcome: A documented opinion on ongoing treatment, with the rationale for continuation, modification or cessation to support the insurer's funding decision.
