What’s New at IMM for 2025? Our recent updates spotlight a major rollout: Medication Risk Assessments are now being integrated across all claims, particularly those entering the 12 to 26-week stage. This is about smarter intervention—earlier, targeted, and pharmacist-led.

Why Follow-Ups Matter Too often, doctors agree to change something—but the change never happens. Whether it’s patient resistance, clinical uncertainty, or lack of follow-through, without a structured follow-up, momentum is lost.

A one-off assessment doesn’t fix a medication issue. Follow-ups ensure:

  • Doctors are accountable for agreed changes
  • Barriers are addressed early
  • Medication plans stay on track

We typically recommend follow-ups at 3, 6, or 12 months, depending on the case. A 3-month window often meant the patient has been seen by their GP 4–6 times within this period, ample opportunities to implement pharmacist recommendations.
